I'll start with the tortured execution of William Wallace in Braveheart. The public castration and disembowelment, his final word, his beheading.
Mel Gibson played it well. That was a tough one for me. (Post your scene please)

The Exorcist - the entire movie.
 
The hills have eyes was pretty bad in the back of that RV.
 
The Green Mile. For my money, the closing monologue (wrapping with "sometimes, the Green Mile seems so long") is the hardest. But strong arguments for a few other scenes in that movie.

Reservoir Dogs - Stuck In The Middle with You.

The Mist - THAT scene. The one that made you want to take Frank Darabont and go back in time to kill his great grandfather, even knowing that it would come at the cost of Shawshank and The Green Mile. But even temporal scrubbing it into non-existence can't make you unsee it.
 
Sticking with Mel Gibson, the Mayan Human sacrifices at the Mayan temple in Apocolypto.

It's an astonishing movie in its scope. The whole Mayan city scene is incredible, the amount of extras and the world it portrays.
